TB-500 (Thymosin Beta-4): A Scoping Review of the Evidence

TB-500 is a synthetic peptide related to Thymosin Beta-4 (Tβ4), a naturally occurring 43-amino-acid protein found throughout the human body. It is one of the most frequently researched “repair peptides,” largely due to its involvement in angiogenesis and cell migration pathways.

What It Is

Thymosin Beta-4 is the most abundant member of the beta-thymosin family in humans. TB-500 is the name commonly used for synthetic peptide products related to Tβ4, though the precise definition of “TB-500” varies across commercial and academic sources, which makes cross-study comparison difficult.

Proposed Mechanism of Action

Across the literature, the most consistently reported mechanistic themes are the promotion of angiogenesis (new blood vessel formation) and cell proliferation, alongside inhibition of apoptosis (programmed cell death) and inflammation. These pathways are shared by many of the tissue types studied.

What Preclinical Research Shows

A recent scoping review mapped the existing literature across tissue categories, in order of research volume: wound, skin and soft tissue (22 studies), vascular and endothelial tissue (17 studies), ocular and corneal tissue (8 studies), and bone (7 studies). Cartilage, ligament, tendon, muscle, and spinal-disk research remain comparatively sparse, with only 1–4 studies each. Study designs were split between mixed designs (33.8%), in-vitro work (28.8%), and animal studies (13.8%).

Human Clinical Evidence

Human evidence is markedly limited and concentrated almost entirely in ocular/corneal and wound/skin settings. Direct human evidence specifically for “TB-500” (as opposed to endogenous Tβ4) was limited to a single included study in the most recent scoping review, and no human interventional studies exist for direct musculoskeletal applications such as tendon, ligament, muscle, cartilage, or bone.

Safety & Regulatory Notes

The FDA has listed the Thymosin Beta-4 fragment among bulk substances presenting “significant safety risks in compounding because of limited human exposure and safety data.” The World Anti-Doping Agency (WADA) classifies Tβ4 and its derivatives as prohibited substances in competitive sport. Products distributed outside regulated pharmaceutical pathways may vary considerably in composition and purity.
